Request a Transcript

To obtain an official transcript to document your academic record, please send a written, signed request with a check for $15.00 (for each copy requested) to the Administration Office at your home campus. Be sure to indicate the address to which you would like us to mail the transcript(s). You are also welcome to make your request via fax or in person. However, we do not accept cash payments.
